MH-17 Dutch Safety Board Report

Both the cockpit and the cockpit roof were found nearest the last known recorded position of the aircraft, while the main fuselage, wings and tail were furthest from that position.  This is the first indicator of attack by means other than a ground-launched guided missile.  The second indicator of attack by aircraft is the concentration of projectile holes in the forward section.  SAM missiles target the heat signature of the engines.

This is the remains of the cockpit floor.  Notice the circled indentations and holes–all indicating an attack from above.  If the cause of the many holes was a SAM missile from the ground, the holes would have been from beneath the floor.
